Making an appointment
Please call us at (804) 639-0400 to schedule your appointment. We are available to assist you Monday through Friday from 9:00 am until 5:00 pm.
Here you can request a follow up appointment only and an associate from HMG will call you to confirm or reschedule.
Changing your appointment
Please call us at (804) 639-0400 at least 24 hours in advance to change or cancel your scheduled appointment and to avoid a no show fee.
Preparing for your first appointment
Thank you for choosing HMG Psychiatrics. We want to make sure your first appointment goes smoothly. Here is some information to help you prepare for your first visit:
Paperwork
You may want to print and complete your paperwork at home and bring the completed forms with you to your appointment. Or, be sure to allow time to fill out and for the staff to process the necessary intake forms for your initial visit. This will facilitate and help ease the stress of this important event in your life. Include any medical and psychiatric records.
Remember to bring
Any of the forms that you completed at home
Your insurance card
A government issued ID photo
Insurance referral or pre-authorization, if required
Your co-payment. We gladly accept cash, and most major credit cards. Personal checks are not accepted.
Any medical records, lab results, drug screens or other pertinent medical history. Preferred pharmacy phone number.
Prescription or over-the-counter medications, along with a list of current medications.
Your Arrival
Plan to arrive 30-45 minutes prior to your appointment time. If we have snow, ice, or other extreme weather, you may want to call our office first to confirm your appointment. Every effort will be made to make your visit as smooth and comfortable as possible.
